1. Termination and follow up

The main objective is to ensure normal pregnancy with delivery of healthy baby from a healthy mother.

In first visit, screen out high risk pregnancy cases, health status of mother and fetus, and patient desire to terminate pregnancy. A midwife should help to educate the situation and enable the pregnant women to decide what is right for her and her coming baby in existing situation.

After carefully examination and counseling advised for follow up according to complications .

2. Intervention

Remove unknown fear by talking sympathetically and improve psychologically by explaining the phenomenon of pregnancy and labor by providing assistance to ho through it successfully.

Provide information regarding nearest integrated counseling center and health facilities and motivate her to tie up for future services.

Provide information on sex during pregnancy.

3. Setting Goals

Help the women to plan and prepare for birth which include decision for place of delivery, person to conduct delivery.

Advantages of institutional delivery and risk involved in home delivery.

Advise the women on where to go if any emergency arises and how to arrange blood donor in case of any emergency.

Advise the nutritious diet , rest and immunization.

Emphasize the importance of breast feeding.

She has to formulate a plan of subsequent management .

4.Assessment

  • Checking for symptoms of poor nutrition or lack of iodine. …
  • Checking her weight. …
  • Checking her temperature. …
  • Checking her pulse. …
  • Checking for signs of anaemia. …
  • Checking for shortness of breath. …
  • Checking her blood pressure.

5. Establishment of relation

A midwife should make rapport with pregnant lady during home visit or antenatal clinics with special contact arranged for particular purpose. She has to be friendly, intelligent approachable. It would be wise for midwife to listen observe carefully without interruping her .
